Announcing a New Tool For Your New and Repeat CheckMate Orders

In support of your existing PO, you can choose to use our new CheckMate Order Form. The form is available to
download from our website (click
click here to download now), or can be ordered in pads from any of our 3 manufacturing facilities.
After completion, you can fax, scan and email (only the face is necessary) or mail the order to us.
We developed this tool for your
benefit as it provides you all the information relevant to our CheckMate program as it guides you towards determining the
specifications you require, and greatly minimizes the chances for misinterpretations. This tool is an option for your consideration and
its use is NOT mandatory.
